Q. Do I put insulation under my grade beam in a heated garage or just on the outside and under the floor.

A. You only need to put insulation around the perimeter, at least on foot vertically and 3-4 ft horizontally. The ground under the garage floor will remain a constant temperature(around 50-55 degrees) throughout the year. You're just throwing away money by insulating the entire floor, as well as compromising the strength of the floor.
